BEFORE downloading the application form, please read the following:
You can complete the application form on your computer/tablet screen before printing. Doing so increases the accuracy of the information being inputed into our computer. We don’t have to worry about being able to read someone’s handwriting.
- Download the form below to your desktop. This makes it easier for find.
- Open the form with Adobe Reader (a FREE program which is probably already on your computer or tablet).
- Click/tap inside the first field (Club Name or Independent)
- Start typing.
- To get to the next field, press the tab key or click/tap on the next field and start typing.
- When all finished, print the document.
- After printing, you will have to check off all the boxes that apply.
- Sign and date the application before mailing.
DON’T FORGET TO INCLUDE YOUR CHECK.
